As a parent, you play a key role in your student's health and wellness.  Below are information and links to assist parents and students.  Questions, call: Candace Winstead, LVN (254)765-3523

At the beginning of the school year

Provide the school nurse with documentation of your child's up-to-date immunizations. The documentation must include the month, day, and year of the immunization.

During the school year

Please keep your child's Emergency Card information up-to-date (contact phone numbers, allergies, the name of the doctor, etc.).

At the end of the school year

If your child's medication is stored at the school clinic, you must pick it up before the last day of school.  Any unused medication not picked up will be destroyed at the end of the school year.

Consent to Communicate with Medical Health Care Professional or Health Care Provider

This is a new regulation regarding the consent to contact your child’s health care provider if needed, in order to better provide school health-related services. In order to begin or continue to provide this service to your child, this consent must be on file. The regulation is provided in the form below. Please sign and return this document to the school nurse, so we can provide continuity of care to your child.

** Aurora Concepts will be here April 30th offering immunizations to incoming Pre-K and Kindergarten as well as immunizations for 6th graders going into 7th and Juniors/Seniors.
